Bonfire Coaching

Client Calls Coach & Arriving On Time: Client calls the coach at the scheduled time or for one-on-one appointments, arrives in time to begin coaching session at the scheduled time at our offices, and is 100% present for the session. _______

Fees Paid In Advance & Missed Appointments: Client pays coaching fees in advance. Any missed appointments without a 24 hour in advance notice count toward one’s package of services. _______ Client's account will be billed for an additional session for every hour of time over the original time allotted. If over by a half hour, a half session will be charged to the client's coaching plan. I understand that my session usage is at 25 and 50 minute increments. _______.

Minimum Usage of Sessions: ( except for Executive Coaching ) There is a minimum use of 2 sessions per month in your plan. You can roll over 1 session per month for future use. If less than 2 sessions are used in one month, the unused sessions will be forfeited and applied to our scholarship program. _______

Coaching Relationship & Business/Career/Personal Coach Role: I understand that coaching is a relationship with a coach that is designed to facilitate the establishment of long-range goals and short-term objectives and the achievement of those goals. _______ I understand that the role of the business/career/personal coach is to assist me to improve the quality of my life and strategies in my career/business. _______

I understand that my coach is not a recruiter nor placement agency and is not obligated to find me jobs or opportunities. _______

Client Attention and Readiness: I will be 100% present for each session without influence of alcohol, mind altering drugs, or distractions that will take my attention away from the session.  I will not be driving or participating in any activity that requires my undivided attention. I will come prepared for each session.  _______

Fees are Non-Refundable: I understand that paid coaching fees are 100% non refundable. _______ I understand that the coaching contract cannot be paused or put on hold, and that requesting to pause can result in the full collection of payments due. ________

Terminating The Coaching Agreement: Any desire to end our coaching agreement prior to the end of the contracted term will result in the collection of the full remaining balance of payments due. The last payment will be debited from my account on file in the full remaining balance. ______ I understand that payments not collected will be turned over to a collection agency after 60 days of non-payment. ______

Coaching is Comprehensive & Client Responsible for Decisions: I understand that coaching is comprehensive in that it deals with almost all areas of my life including work, finances, health, education, relationships, and spiritual issues. I acknowledge that deciding on how to handle these issues and implementing my decisions remains my exclusive responsibility. _______

Client Confidentiality: I understand that the confidentiality in the coaching relationship is strictly confidential. Confidentiality will not apply to certain crimes that have either been committed or are planning to be committed. Your Coach is accountable by law to report such crimes to legal authorities. _______

Coaching Not Treatment for Mental Disorders: I understand that coaching does not treat mental disorders as defined specifically in the DSM-IV. If I have anything in my past indicating that I have an unresolved and serious emotional or physical problem or a mental disorder, then I certify that I am not using coaching as a substitute for assistance from a mental health professional or a medical doctor. _______

Coaching Not Substitute for Professional Counseling: I will not use coaching as a substitute for professional counseling, psychotherapy, psychoanalysis, mental health care, or substance abuse treatment. _______

Acknowledgement of Therapy or Mental Health Professional: If I am currently in therapy or under the care of a mental health professional, I will have consulted with that person regarding the advisability of my working with a life coach. Additionally, I will inform my coach of this relationship in writing. _______

Coaching Not Professional Advice: I will not use coaching in lieu of professional medical advice, legal counsel, accounting assistance, or spiritual guidance and for each of these areas I understand I should consult the appropriate professionals. I acknowledge that I will not use coaching as a substitute for such professional guidance. I further acknowledge that all decisions on dealing with these issues lie exclusively with me. _______

Agreement for Regular Evaluations & Reporting Concerns: I agree to complete regular evaluations of the coaching process and notify my coach IMMEDIATELY of any concerns. _______ I agree to report any issues I may have with my coach to the administration office immediately by calling 888-721-4204 or 713-332-3858. ________
